When you request an appointment on our website, as part of the process, we collect the personal information you provide, such as your name, identification number, and email address. Your personal information will be used solely for the purpose of properly identifying you to process your request.

Our company is hosted on the Wix.com platform. Wix.com provides us with the online platform that allows us to offer you our services. However, your data is processed by a third-party provider and may be stored via Hushmail's data storage. They store your data on secure Canadian servers in accordance with Quebec's Act respecting the protection of personal information in the private sector (Act 25).

We may contact you to update you on the status of your request or to obtain further information. For these purposes, we may contact you by email or telephone. If you no longer wish us to process your data, please contact us by telephone at 450-955-3442.

We reserve the right to modify these terms of use at any time, so please review them frequently. Changes and clarifications will take effect immediately upon posting on the website. If we make significant changes to this policy, we will notify you here that it has been updated, so you are aware of what information we collect, how we use it, and under what circumstances, if any, we use and/or disclose it.

Our practice complies with Quebec's Act respecting the protection of personal information in the private sector (Bill 25). The designated person responsible for ensuring compliance within the practice, as well as for managing and recording confidentiality incidents, is Dr. Jérémy Claes, physician and owner.
